Events Forms and Procedures

The Mountview Event Procedure Checklist is applicable for non-regular activities (eg. BBQ/Campfire dinner, Overnight camp/bushwalk, Mountain Bike youth ride, Gingerbread house event, Marriage Course, Holiday Kids Club, Youth Group off site, KYCK youth weekend, Carols, Church families weekend camp, etc). The procedure is not applicable for regular activities (Church services, Sunday School, Youth Group on site, MOPS, Growth Groups, Newish, Retired Men’s lunch, Ladies’ breakfast, etc) as their risks have been previously assessed and managed consistently.

Subject to whether a non-regular event is assessed as Low, Medium or High Risk (based on the Hazard Analysis), the following forms need to be completed before seeking Mountview Session/COM approval of the Event, Risks and Costs:

  • Low risk events require ‘Hazards Analysis’ and ‘Church event WHS checklist’
  • Medium to High risk events require ‘Hazards Analysis’, ‘Church event WHS checklist’, ‘Risk Warning Forms B or A&B’ for all Adult participants and Parents for under 18’s, ‘Pre-event safety brief’.
